Did you lose your vaccine card? Don’t worry! You can get it replaced! See original post below from Pima County Health Department:

Did you lose your vaccine card? Don’t worry! You can get it replaced!

1. You should first contact the site where the vaccines were received.
2. If the original vaccinator is unable to provide a replacement card, a new card can be obtained from the Health Department. Someone requesting a vaccine record should send an email to [email protected] that includes name and phone number.
3. If the requestor does not have email access, they can be directed to the main HD phone line (520-724-7770) where a staff member will get their name and phone number. This information will be sent in an email request to [email protected]. Phones will be staffed Monday-Friday 8 a.m. to 5 p.m.

Requestors will also be offered access through MyIR. MyIR is an application that gives access to the Arizona State Immunization Information System (ASIIS) vaccine data for printing and downloading. The use of MyIR requires verification from a clinical provider. Registration in MyIR will not be required to get a replacement vaccine card.

Our Story

Desert Senita Community Health Center provides primary health care services in Ajo and Arizona City.

Desert Senita has evolved from a hospital and outpatient facility which was run by a large mining company until it ceased local operations in 1986. Following the mine closure, the hospital closed and the outpatient treatment facility relocated to the mine’s former single men’s dormitory, in a more central area of the Ajo community. The health center was then run by hospital management organizations hired by the mining company for a few years, prior to becoming an independently-run facility in the late 1990s.

Desert Senita has been an independent, outpatient treatment facility since 1997 and became a Federally Qualified Health Center (FQHC) in 2001. The health center provides health care for the entire family, including preventative and primary health care services, dental services, behavioral health services, outreach and education programs, and eligibility assistance for the Sliding Fee Discount Program and AHCCCS programs. In January 2010, Desert Senita added in an in-house pharmacy for our patients’ convenience, which is in the process of a remodel. In 2017, Desert Senita expanded its services to Arizona City, Arizona. Today, we are in the stages of remodeling our facilities to provide a better patient experience.
